Hypertension Drugs


With Congestive Heart Failure:

ACE Inhibitors

Benazepril (Lotensin)

Enalapril (Vasotec) Lisinopril (Prinivil, Zestril)

Quinapril (Accupril)

In HF, ARBS may be combined with neprilysin inhibitor: Sacubitril


Beta blockers (Compensated HF)- contraindicated in cardiogenic shock

Atenolol (Tenormin)

Bisoprolol (Zebeta)

Nadolol (Corgard)

Nevibolol (Bystolic)

Propanolol (Inderal, InnoPran XL)

Aldosterone antagonists

Aldactone

CaroSpir

Eplerenone

Finerenone

Inspra

Thiazide Diuretics

Are used in osteoporosis since they hold on to calcium

With Diabetes:

ACE Inhibitors (protective against diabetic nephropathy)

ARBS

Calcium channel Blockers


Dihydropyridines:

Amlodipine (Norvasc)

Felodipine

Nimodipine - used for subarachnoid hemorrhage (prevents cerebral vasospasm)

Nicardipine

Nifedipine (Procardia)

Nisoldipine (Sular)


A/E of -dipines: Gingival hyperplasia and reflex tachycardia


Diltiazem (Cardizem, Tiazac)

Verapamil (Calan SR, Verelan) A/E: Constipation

Thiazide Diuretics

Chlorthalidone (Clorpres, Tenoretic, Thalitone)

Hydrochlorothiazide (Capozide, Diazide, Hyzaar, Lopressor HCT)

Beta Blockers

A/E: sexual dysfunction, cardiovascular depression, , fatigue, Increase LDLs and TGs


In Asthma:


ARBS, Calcium channel blockers, thiazide diuretics

Avoid ACE inhibitors to prevent confusion between drug or asthma related cough

Selective Beta- 1 blockers

Atenolol, Bisoprolol

Esmolol (Brevibloc)

Acebutolol (Sectral)

Metoprolol (Lopressor, Toprol XL)

Nebivolol


Avoid non-selective Beta blockers to prevent Beta- 2 receptor induced bronchoconstriction

In Pregnancy:


Hydralazine (Apresoline, BiDil)

Vasodilates arterioles > veins by increasing cGMP, causing smooth muscle relaxation. Reduces afterload

Co-administered with a Beta-Blocker to prevent reflex tachycardia

A/E: Fluid retention, angina, SLE-like syndrome, compensatory tachycardia

C/I in angina and Coronary artery disease

Labetalol (Trandate)

Blocks alpha and beta receptors

Causing decreased peripheral vascular resistance without affecting the heart

Methyldopa (Aldomet)

Binds to alpha 2 receptors as an agonist, reducing adrenergic neural outflow, and reducing vasoconstrictor signals

Causes Vasodilation

Nifedipine (Procardia)

Calcium channel blocker: helps to relax uterine contraction and helps postpone preterm births
